A 23-year-old Mexican mother was crushed to death in front of her daughter while attempting to lift a 400lb (180 kg approx) barbell.

Ayesha Lemus broke her neck and suffered serious internal injuries, and died at a hospital in the city of Puebla, after falling on gym equipment while attempting to break a record.

Surveillance cameras at the CrossFit gym in Peralvillo, Cuauhtemoc, in Mexico City, captured the shocking episode on Monday, Feb 21.

The footage shows a man, probably an instructor, adjusting the weights on the barbell before the woman sits beneath it.

She attempts to lift the weights but fails, the bar falls on the back of her neck and pins her down to the seat.

The man and the young girl, reportedly her daughter, rush to help but their attempts to lift the barbell also go in vain.

Two other people then joined and helped lift the weight off the woman. Unfortunately, it was already too late at that point. The mother can be seen falling lifelessly onto the floor.

The woman was reported by local media to be aged between 35 and 40 years old.

Her daughter, who witnessed the accident, “is said to be receiving psychological support after the horrific event.”

The death has prompted the state prosecutor’s office to open an investigation into the tragic incident.

The gym owner was reportedly briefly arrested in order to confirm the woman’s identity, the Dailymail reported.

A similar incident was reported last year when a Florida man who was attempting to break a Guinness world record for skydiving reportedly died in a freak accident.

Thomas Noonan III, the chief technical director of Everest Skydive, who has several certifications as a United States Parachute Association (USPA) tandem instructor and advisor, died at the age of 47 after he lost consciousness due to hypoxia while he was attempting to a tandem jump from 41,000 feet over Memphis, Tennessee.

Noonan and his friend Jim Wigginton, 72, along with two solo jumpers, an oxygen manager, a videographer, a jumpmaster, and the pilot, were about to attempt to break the Guinness world record for the highest tandem skydive when Noonan's foot got stuck while heading out of the plane.

However, before Noonan and Wigginton jumped, the oxygen system malfunctioned and everyone in the plane began to experience hypoxia, a state in which oxygen is not available in sufficient amounts.

However, after some time, most of the crew members apart from Noonan regained consciousness and came out of hypoxia. Noonan and Wigginton were pulled back into the plane and were given CPR.

After the plane landed, Noonan was transported to a nearby hospital but was pronounced dead on arrival. However, his cause of death was not determined.
